
Shockwave Therapy or Cortisone for Tendon Pain?

A sore Achilles that flares with every morning step, a shoulder that hurts when you reach overhead, or an elbow that will not settle can make treatment decisions feel urgent. When comparing shockwave therapy or cortisone, the best option is rarely about finding a single “strongest” treatment. It is about understanding the source of your pain, the condition of the tendon or surrounding tissue, and what will help you return to comfortable, confident movement.
Both options can have a place in care for certain musculoskeletal conditions. They work very differently, though, and neither replaces a well-designed rehabilitation plan. A physiotherapy assessment can help clarify which approach, if either, fits your symptoms, goals, activity demands, and medical history.
Shockwave Therapy or Cortisone: The Key Difference
Shockwave therapy uses acoustic pressure waves applied to a targeted area. In physiotherapy, it is commonly considered for persistent tendon-related pain, including some cases of plantar heel pain, Achilles tendinopathy, tennis elbow, and shoulder calcific tendinopathy. The treatment is delivered in clinic and does not involve medication or an injection.
Cortisone, more accurately called a corticosteroid injection, is medication prescribed and administered by an appropriately qualified medical provider. Its main purpose is to reduce inflammation and pain in selected conditions. It may be injected into or around a joint, bursa, tendon sheath, or other painful structure, depending on the diagnosis.
That distinction matters. Shockwave therapy is generally intended to support a gradual change in stubborn tissue-related pain alongside loading and rehabilitation. Cortisone can offer relatively rapid symptom relief in an inflammatory presentation, but it does not build tendon capacity, restore strength, or correct the movement and workload factors that may have contributed to the problem.
When Shockwave Therapy May Be Considered
Shockwave therapy is usually considered when pain has persisted despite appropriate initial care, particularly where a tendon has become sensitive to load over time. Tendinopathy is not always a simple inflammation problem. In many cases, the tendon has reduced tolerance for the demands placed on it, whether that means running, repetitive lifting, prolonged standing, or returning to sport too quickly.
During treatment, you may feel a tapping or pulsing sensation over the affected area. The intensity can be adjusted to remain tolerable. A course often involves several sessions spaced over a number of weeks, paired with exercises that progressively rebuild strength and loading tolerance. Improvement is typically gradual rather than immediate.
Shockwave may be a reasonable option for someone with long-standing plantar heel pain who has not improved with activity modification, footwear guidance, and exercise alone. It may also help an active adult whose chronic elbow or Achilles symptoms are limiting work, training, or daily tasks. The exact type of shockwave therapy and treatment plan should be selected based on the condition and clinical findings.
It is not appropriate for every painful area. Certain medical conditions, medications, pregnancy considerations, blood-clotting concerns, or treatment locations may affect whether it is suitable. A clinician should screen for these factors before proceeding.
When Cortisone May Be Considered
Cortisone may be considered when inflammation is a significant driver of pain and symptoms are preventing sleep, work, or participation in rehabilitation. For example, a person with severe shoulder bursitis or a painful arthritic joint may gain a useful window of relief from an injection. That relief can make it easier to start moving, strengthening, and returning to necessary activities.
The timing and location of the injection are important. A cortisone injection may provide short-term pain reduction, but results vary, and the effect can wear off. In some tendon conditions, repeated steroid injections or injections placed too close to a tendon can raise concerns about tissue weakening. This is one reason an injection should be a considered medical decision, not an automatic answer to pain.
Cortisone is also not a substitute for rehabilitation. If pain drops but the underlying strength, mobility, technique, workload, or recovery habits do not change, symptoms can return when normal activity resumes. The most productive use of short-term relief is often to create an opportunity for focused physiotherapy.
How to Decide Between the Two
The choice between shockwave therapy and cortisone starts with an accurate diagnosis. Pain at the outside of the hip, for instance, can involve the gluteal tendons, a bursa, the lower back, or a combination of factors. Treating the wrong structure may delay progress, even if the treatment itself is well delivered.
A physiotherapist will look at when symptoms began, what aggravates them, how they behave over a full day, and what movements reproduce them. They may assess joint mobility, muscle strength, tendon loading tolerance, balance, walking or running mechanics, and the demands of your job or sport. This fuller picture helps determine whether your pain appears more inflammatory, tendon-related, joint-related, or referred from another area.
In general, shockwave therapy may be more relevant when:
symptoms have been present for months and behave like a persistent tendon problem;
progressive exercise has helped only partly or has plateaued;
the goal is to support longer-term recovery without an injection; and
you can commit to the gradual loading programme that follows treatment.
Cortisone may be more relevant when pain and inflammation are severe enough to block sleep, daily function, or rehabilitation, and a medical provider identifies an appropriate injection target. It can also be considered when other conservative approaches have not provided enough relief. The decision should include a clear plan for what happens after the injection, not simply a plan to wait and see.
Why Rehabilitation Still Matters Most
Whether you receive shockwave therapy, cortisone, both at different stages, or neither, rehabilitation remains central to durable recovery. Pain is only one part of the picture. Your body also needs the capacity to tolerate the tasks that matter to you, from climbing stairs and carrying children to completing a shift or returning to the gym.
A personalized physiotherapy plan may include hands-on treatment to address mobility restrictions, targeted strengthening, gradual tendon loading, movement retraining, and advice on pacing your activity. For a runner with Achilles pain, that may mean adjusting mileage while building calf strength. For someone with shoulder pain, it may mean restoring shoulder blade control and rotator cuff capacity before returning to overhead lifting.

It is also reasonable to ask what happens if the first approach does not help.
Seek prompt medical assessment for severe pain after a sudden injury, an inability to bear weight or use a limb, a suspected tendon rupture, marked swelling or redness, fever, unexplained numbness or weakness, or pain that is worsening rapidly. These symptoms may need a different level of care than routine tendon treatment.
The right choice is the one that fits your diagnosis and gives you a practical route back to movement. A thoughtful assessment can turn a frustrating choice between treatments into a clear recovery plan that supports the way you want to live, work, and move.
